Florida Blue, in partnership with MCPS and Advent Health Ocala, is pleased to offer the MCSB Diabetes Management Program. Designed to provide resources and education to help diabetics effectively manage their disease, the Diabetes Management Program is open to employees, spouses, and adult dependents who are currently enrolled in the school board's health insurance plan.
As part of the program, participants will receive the following services at no cost:
- Diabetes and Nutrition Education provided by Advent Health's Diabetes Education Center (10 hours)
- Diabetic Screenings (HbA1c, Lipid Profile, Microalbumin)
- Diabetic Podiatry Exam
- Dilated Retinal Exam
In addition to the initial education classes and screenings, participants will be monitored throughout the program.
Once a participant has completed the program requirements, they become eligible for several valuable incentives:
- Diabetes, Blood Pressure, and Cholesterol Medication (GENERIC ONLY)
- Insulin
- Testing Strips
- Lancets
- Needles/Syringes
