Please read the following information before proceeding to register your students.

Student Immunization: Upon enrollment, as part of The Department of Children and Families licensing requirements, each child must have a complete and properly executed Florida Certification of Immunization Form or the Religious Exemption Form. This includes PK3, PK4, VPK.

This form should be turned in to the front office at Fordham Early Learning Academy to the attention of Pam Eubanks. It is the parent's responsibility to submit updated immunization records as your student receives immunization throughout the year.

Student Health Record: Upon enrollment, as part of The Department of Children and Families licensing requirements, each child must have a complete and properly executed Student Health Form. The physical must be within one year of enrollment. This form should be turned in to the front office at Fordham Early Learning Academy to the attention of Pam Eubanks. It is the parent's responsibility to submit updated immunization records as your student receives immunizations throughout the school year.

Department of Children and Families Brochures: All parents of incoming students will be required to sign a statement that they received a copy of Know Your Child Care Facility Brochure.
In the months of April and September, you will receive additional brochures that will require your signature as part of the DCF licensing requirements.
